Facility fee
operating and recovery room services & supplies for outpatient stay (less than 24 hours) at Bradley Medical Center
Physician fee
treatment and procedure-related postoperative follow-up visit(s) within 90 days of surgery. Does NOT include the initial evaluation to determine the need of the procedure.
Anesthesiologist fee
regional or general anesthesia for the entire duration of the procedure
Pathology fee
specimen(s) examination and tests for diagnosis. This test does not include the pathologic workup of malignancy. If malignancy is discovered during your procedure, additional charges will be incurred.
MDsave fee
Covers the cost of contracting upfront rates with all involved medical providers, processing payment, and distributing funds, minimizing administrative overhead and allowing for a lower total price

Dr Wolf has been in the practice of OB/GYN for 16 years. He obtained a Bachelors of Science in 1977 from Ohio State University. He then received his Masters of Healthcare Administration in 1980 from Ohio State University. He received his Doctor of Osteopathy in 1984 from the Ohio University College of Osteopathic Medicine. He completed his Osteopathic Internship in 1985 and his Family Practice Residency in 1986 from Botsford General Hospital in Farmington Hills, Michigan.
