Our board-certified family medicine providers will take care of your acute care needs, and provide preventive and well care services. A diabetic educator is available as well. Lab tests, if needed are available on site. Fairmount Family Medical Care also offers patients convenient access to behavioral health specialists in the same facility. Our physicians will arrange for referrals to specialists as needed, and coordinate care through medical specialists affiliated with TMC Lakewood. Medicare Initial Preventive Physical Exams ( IPPE) and Annual Well Visits (AWV) are encouraged for Medicare eligible patients and a covered benefit once a year. These visits allow your physician to review your current medications and schedule your well care and screening visits. When you come in for acute care needs, your history is up to date and the focus can be on that immediate need. Appointments can be scheduled Monday through Friday.
