procedure fee for in hospital service. Does NOT include the initial evaluation to determine the need of the procedure. If you have not previously seen this provider, please purchase a New Patient Office Visit.
MDsave fee
Covers the cost of contracting upfront rates with all involved medical providers, processing payment, and distributing funds, minimizing administrative overhead and allowing for a lower total price
