Facility fee
operating and recovery room services & supplies for outpatient stay (less than 24 hours) at Adventist Health Glendale
Physician fee
physician supervision & treatment in hospital. Does NOT include the initial evaluation to determine the need of the procedure.
Anesthesiologist fee
regional or general anesthesia for the entire duration of the procedure
Pathology fee
specimen(s) examination and tests for diagnosis. This test does not include the pathologic workup of malignancy. If malignancy is discovered during your procedure, additional charges will be incurred.
Tendo Marketplace fee
Covers the cost of contracting upfront rates with all involved medical providers, processing payment, and distributing funds, minimizing administrative overhead and allowing for a lower total price

She treats every patient as a unique individual, identifying their particular needs and developing a tailored approach for each patient. She is committed to delivering highquality care and believes that aligning with Adventist Health Glendale will allow her to continue to do that.
With more than 30 years of experience in this community, Dr. Chan’s goal is to ensure that women can live their healthiest lives possible, through all stages of life. And, when they choose to be moms, she is there to help them bring their babies into the world with a warm welcome.
