Your purchase includes the following services:
Physician fee
Vaginal delivery with or without cervical dilation, episiotomy and/or forceps, and your postpartum visits during your stay in the hospital.
Facility fee
1-3 days stay in the hospital for the mother and ONE well newborn. Length of stay in the hospital will be the decision of your physician based on your medical needs. If postpartum complications occur, the mother or the newborn may be required to stay longer in the hospital and there will be an additional charge for each day.
Anesthesia fee
This fee is for either local anesthesia, an epidural, or a spinal block. It also includes any repeat subarachnoid needle placement and drug injection and/or any necessary replacement of an epidural catheter during labor. If you choose to not undergo anesthesia, you will be refunded the anesthesia fee.
Pediatrician fee
This DOES NOT INCLUDE the exam performed by a pediatrician. You will receive a bill for this portion.
Hospitalist fee
DOES NOT INCLUDE THE HOSPITALIST FEE. Hospitals often contract with hospitalist groups who provide care to patients if the primary care physician becomes unavailable. If your situation requires hospitalist services, you will receive a bill from the hospitalist group.

Covers the cost of contracting upfront rates with all involved medical providers, processing payment, and distributing funds, minimizing administrative overhead and allowing for a lower total price

Hobbies & Interests
Dr. Jakacki loves football and is a big Indianapolis Colts fan. He's also an avid gardener and appreciates the good results from eating organic, natural foods. With his family, he grows his own cucumbers, tomatoes, zucchini, onion, broccoli, brussel sprouts, radishes, parsley, squash and more. Dr. Jakacki is a man of faith and lives to fulfill God's purpose for his life.
History
A native of Detroit, Dr. Jakacki became a member of Lutheran Health Physicians in August 2010 when his former group, Brooklyn Medical Associates joined LHP. A family medicine physician, Dr. Jakacki joined BMA in 2001. Prior to that, he worked as an emergency room physician at Lutheran Hospital, and as a full-time chiropractor in private practice.
Dr. Jakacki attended Kalamazoo College in Kalamazoo, Michigan, in 1979 and the University of Michigan in Ann Arbor from 1980 to 1982. He received his Bachelor of Science in Human Biology degree in 1986 and a doctor of chiropractic degree from The National College of Chiropractic, now known as National University of Health Sciences, in Lombard, Illinois, in 1988. He earned a medical degree from the Indiana University School of Medicine in 1998, and he completed a family medicine residency at the Fort Wayne Medical Education Program in 2001. He graduated at the top of his class in both chiropractic college and medical school.
Dr. Jakacki is board certified by the American Board of Family Medicine and is also board certified in functional medicine. He is a member of the American Academy of Family Physicians, the American Medical Association, the Institute of Functional Medicine, the American Society for Colposcopy and Cervical Pathology, the Christian Medical Dental Society, the Indiana Academy of Family Physicians, the Indiana State Medical Association and the Fort Wayne Medical Society.
