Center for Diabetes Control CATAWBA COUNTY DIABETES MANAGEMENT   Accredited by the American Diabetes AssociationThrough the Center for Diabetes Control, your diabetes healthcare team is made up of you, your physician, a nurse, a dietitian and a pharmacist, all of whom have received the most up-to-date training in the most current methods of treating diabetes. Most clinical staff members of the Center hold Certified Diabetes Care and Education Specialist (CDCES) status from the Certification Board for Diabetes Care and Education (CBDCE) – which means they possesses distinct and specialized diabetes-related education, thereby improving the quality of care for patients with diabetes.   If you have questions or concerns about diabetes, The Center for Diabetes Control at CVMC can help. Call 828.326.3442 or email us at DiabetesEd@cvmc.us for more information.   Diabetes Self-Management Training   Knowledge and understanding are the most important tools in the fight against diabetes. That’s why we offer a comprehensive education program to help you learn ways to control and manage the effects of the disease. In either a classroom or one-on-one setting, you can learn about these vital topics:   Nutrition Therapy Exercise Blood Glucose Diabetes Medications Prediabetes Stress Management Complications Associated with Uncontrolled Diabetes Sick Day Management   Prediabetes   Prediabetes is a blood sugar level that is higher than normal but not high enough to be diagnosed as diabetes. One in three American adults has prediabetes, and most do not even know they have it.   If you have prediabetes or are at risk for developing type 2 diabetes, now is the time for prevention! CVHS’ Prevent T2 lifestyle change program is an evidence-based program that can help you take charge of your health to prevent or delay type 2 diabetes.    As part of a PreventT2 group, you will work with other participants and a trained lifestyle coach to learn the skills you need to make lasting changes. These changes include weight loss, being more physically active, and managing stress. Being part of a group provides support from other people who are facing similar challenges and trying to make the same changes you are. Together you can celebrate successes and find ways to overcome obstacles.   This program is free to participants!     Gestational Program   Gestational diabetes is usually a temporary condition that disappears after the baby is born, but it is very important to keep a close eye on it during pregnancy. To help ensure proper care for both mother and baby, our Gestational Program offers specific education on such topics as blood sugar management, meal planning and medication use, if necessary. Your physician will determine if the Gestational Program is needed for you.     Nutrition Therapy   In our Nutritional Therapy program, you will receive one-on-one counseling with a registered dietitian who will help devise a meal plan specific to your personal needs. You will be provided with easy-to-follow guidelines about what you can eat, when you can eat it, and how it will affect your blood sugar levels.   Nutrition and diabetes classes are held throughout the month to meet your scheduling needs. Please call 828.326.3442 or e-mail us at DiabetesEd@cvmc.us to schedule your first class today.
