Physical, Occupational and Speech TherapyTo Access the ClinicPark on any level of the main parking garage and use the “MOB” elevators that are located in the northeast corner of the parking structure. Take the MOB elevator to the 5th floor and exit. You may also use the free valet service that’s located at the main entrance of the hospital. Once you’ve dropped off your car, enter the northwest entrance and walk past NorthStar Pharmacy (on your right). The MOB elevators will be on your left. Appointment PreparationYou’ll want to come prepared to your first appointment. Arrive about 15 minutes before your evaluation, there will be additional forms to fill out to give our therapists a better idea about your current condition.Please bring your insurance card, photo identification (example: driver’s license) and a list of any prescription medications you’re currently taking.Arrive wearing comfortable clothing (or bring a pair of clothes to change into). You’ll want to wear garments that will allow us to easily access the part(s) of your body that are being treated.
