May require a physician's order, which can be faxed to the provider or brought to the appointment unless required when scheduling. Please contact your insurance company and provider for specific information about your out-of-pocket costs and whether or not a physician’s order is required.
Price Details
Your purchase includes the following services:
Facility fee
collection of the sample (blood draw, urine collection, stool sample, etc.) and testing. * CPC Billing Fee not included. You may recieve a separate bill for this fee.
MDsave fee
Covers the cost of contracting upfront rates with all involved medical providers, processing payment, and distributing funds, minimizing administrative overhead and allowing for a lower total price
