Important Note on COVID-19 Testing
May require a physician order, which can be faxed to the provider or brought to the appointment unless required when scheduling. Many insurance plans cover the cost of COVID-19 testing, even for out-of-network providers, and most insurance covers testing costs without imposing any cost sharing obligations (including deductibles, copayments and coinsurance). Additionally, a number of providers have waived the cost of COVID-19 testing. Please contact your insurance company and provider for specific information about your out-of-pocket costs.

Facility fee
collection of the sample (blood draw, urine collection, stool sample, etc.) and testing.
