- Review child's medical history
- Conduct a physical examination
- Offer information on signs of concussion (sports physical)
- Provide a summary document of the visit to the child's primary care provider
- Complete forms needed for sports or school participation (forms to be provided by parent/guardian)
Immunizations are not included in the office visit cost. Separate charges would apply for any additional service(s) and they would need to be purchased separately.
